     Please fill out questionnaire with as much information as possible - your responses help me support you, your unique situation, your goals, desires and to align my energetic transmissions for your project while you
    are in my Queendom and Sacred Container. 
    By submitting this Intake Questionnaire, I understand it is my responsibility to do the work and get the most out of this Method and time. I also understand that there is no guarantee of completion; although it is possible based upon my own situation that I may complete my project.

    I understand I will be asked to reply to check-in questions, and asked for my feedback after each week's assignment so I can benchmark my progress and I am willing to offer my unique perspective and expertise
    during feedback calls at the Queen's Roundtable.

    I understand that I am committing to receiving 4-weeks of coaching and expert guidance, and I will be given the opportunity to continue into the Author Queen Society membership or VIP Day Container if I would like to continue with my support with absolutely no obligation to do so.

    I understand I am about to step into an intuitive and spiritually activated container where
    I will receive transmissions in which my vibration and creativity will be accelerated.
